Plus "Set Menu" Vs Premium "a' la carte"

I live in the US and visting DLP for the first time this coming Easter and super excited. In Disney World we usually buy Tables in Wonderland for the 20% discount and usually just buy what we want when we want vs the WDW Dining Plan. But at least the dining plan is straightforward.
Drink, Entree & Dessert.

But considering it for our upcoming trip, but awfully confused.
The one point that gets me is what can I order?
By the description of the differences between the two plans is:
Plus "Set Menu"
Premium "A' la carte"

Since most restaurants have a full menu and cute groups of set menus.
Both nothing points to what is Plus?

Blue Lagoon has the following
The Pirate's Pearl
Buccanner
Captains Chest

And for Premium.
The California Grill has the following
Tinker Bell Menu
Menu Premium.

Does that mean I can ONLY order from the Menu Premium?

Auberge de Cendrillon seems much easier as there is only one option and I assume Premium covers all options.

For Premium can I order ANY starter, Any Entrée, Any Dessert?

Can they make it any more confusing about what I can order vs what I can't?

Ok, I am getting it a bit better now.
After looking through the menus I see the little symbol next to the options.

If they force you into a set menu the logo is next to the set.
If you can order A la carte it is next to each section, starter, entree, dessert.

So now I have two different questions.

If Premium set is 59.99 and I want to jump up to the next more expensive set menu say 69.99, can I just pay the 10.00 difference?

Next question is, if I am in the premium set but I want to order a starter from a lower set, can I go down for that one option?

Still learning the ropes.

If you go "outside" your available options you simply pay the difference between the value of the voucher and the price of your chosen dishes.
I admit it's a bit more involved than the Dining Plan credits at WDW.

And I think you can select from menus below your plan if you want.
