Where and when do I buy fast passes for a ride?

You don't buy them ;)
You use your entrance ticket (or AP, or if you have a paper ticket you scan the code on it), at the FP's machines that are at certain rides.
You then get an FP-ticket, which has a timeslot on it.
You need to be in the FP-line during that timeslot, in order to use the FP ticket.
Yes, Nathalie is correct. In addition, you can (normally) only have one Fastpass active at a time. So if you were, say to get a fastpass at 1000 for Peter Pan at 1200-1230 you couldn't get another until either you've been on Peter Pan or after 1230. However, I'm told it is possible to get 2 Fastpasses at the same time if they're in different Parcs - I've never tried this though. There are limited Fastpasses for each ride that uses them with BTM and Ratatouille being very popular. There are also VIP Fastpasses for people staying in suites (unlimited Fastpass) and there's additional one-off "hotel" fastpasses for Castle Club and Golden Forest Club guests.

All Castle Club rooms come with a VIP fastpass. Any hotel guest in the DLH, Golden Forest rooms in Sequoia or Empire State in HNY gets a fastpass per person per day to be used on any ride at any time other than early afternoon.

You get the hotel fast passes staying in a Newport Bay Compass Club room as well.
